Check Authorization Form

To pay by check and have the funds electronically paid from your checking or savings account, you will need to print out an Accelerated Electronic Check Authorization form, properly complete the form - noting your online order number which you will receive at the end of this order, and fax it to Dinn Bros. at 1-800-876-7497 along with a copy of your check.

• We do not need a check number; DinnTrophy.com will debit your bank account electronically and the bank does not require a check number for such transactions.
• We cannot begin to process your order until this form has been received along with your completed online order.
• Canadian orders are not eligible to use electronic checks.
• Electronic debits may take up to 48 hours to process (not including our order processing time and shipping time). Please choose your shipping method accordingly to ensure delivery in time for your event.
• Print the Accelerated Electronic Check Authorization form.
